  3. Please include a description of the problem, along with the steps taken to correct it prior to contacting DAC. Please try to reconstruct the problem in a sequential way, such as: "I noticed bad surfaces, so I changed the diamond tools and carefully recalibrated the lathe" etc.
  5. You may include a file to upload to aid support of the problem. This form only accepts: .jpg, .txt, .rar and .zip files to a maximum size of 20MB.
